Baptismal Certificate |First Communion Certificate
Confirmation Certificate | Marriage CertificateTo request Sacramental records or certificates, please complete this form and return to the Parish Office. Requests must be made in writing and with approximate date when sacrament conferred. Record search time is 5-10 business days.

Please be prepared to provide the following information:
Certificate Information.- Type of Certificate: Baptism, First Communion, Confirmation or Marriage
- Approximate month and year the sacrament took place
- Full name of the person the sacrament was for
- Date of birth of the person the sacrament was for
- Mother’s first and maiden name
- Father’s full name
- Your relationship to the person named on the certificate, with a current valid photo ID
- Your email and phone number
- Mailing Address for Parish requesting the sacramental record
In order to protect the confidentiality of these records, the Church can only release sacramental certificates to the individual named on the certificate (or the next of kin if deceased), or to their parent or legal guardian if the named person is under the age of 18.Please note: No certificates will be issued for genealogy purposes.
